We’re looking for someone to join our team to provide holistic support to patients suffering from a serious mental illness (SMI) by helping them access the right support within the community.

The Community Connectors programme is an exciting partnership between:

  • Aston Mansfield
  • Community Links
  • Mind in Tower Hamlets and Newham
  • NHS East London Foundation Trust (ELFT)

After successfully delivering the pilot in 2020, Community Links, Aston-Mansfield and Mind in Tower Hamlets and Newham have been awarded a 5-year contract for the programme launching in June 2022.

About the Role

The role includes a range of duties, which can be found in the Job Description and include:

  • Working within the local community
  • Providing support and signposting
  • Being alert to the mental health needs of residents
  • Work together with a range of partners

About You

You can find everything we are looking for in a candidate in the Person Specification which includes:

  • Degree, qualification or two years relevant work experience
  • Knowledge of issues and barriers faced by people suffering from SMI
  • Experience delivering support and dealing with multiple stakeholders
